Don’t miss the Texas GI Tank at #TexasGI26. 🦈 @KlempnerSam and @rutikamehtaMD go head-to-head on one of the biggest questions in GI oncology: 1L HER2+ Gastric/GEJ – The Role of Immunotherapy and Patient Selection. The sharks—Drs. @YJanjigianMD, Scott Paulson, @GIcancerDoc, and Ryan Corcoran—will put both cases under the microscope before deciding who gets the deal. Don't miss it! The Tank is officially open at #TexasGI26!🦈 Mr. Wonderful himself dropped in to set the stage for our Texas GI Tank debate on 1L HER2+ Gastric/GEJ: The Role of Immunotherapy & Patient Selection. @KlempnerSam is pitching hard that immunotherapy should stay the 1L standard, while @rutikamehtaMD is coming in with the bold counter that bispecifics are the future of 1L HER2+ disease. Waiting in the Tank are four of oncology’s sharpest minds— Drs. @YJanjigianMD, Scott Paulson, @GIcancerDoc, and Ryan Corcoran—who don’t hand out deals easily. Will one strategy land the deal, will the sharks split, or will someone hear the dreaded “For that reason… I’m out”? Join us November 5–7 in Austin at ACL Live and come see which pitch wins. https://t.co/twXDeSwM3F

Enjoyed this session & learned a lot!
Both TARE & SBRT can deliver ablative RT in HCC. Key q is which approach is best suited for a given pt/tumor.
Sharing some of my slides below re: decision-making b/w the 2 modalities.
TY to @GIcancerDoc & @EileenMOReilly for the invite.
Conference cochairs @EileenMOReilly and Colin D. Weekes joined Christoph Benedikt Westphalen and Tanios Bekaii-Saab to discuss "Smarter Clinical Trials and Precision Development" in a plenary session at #AACRpan26. @BenWestphalen@GIcancerDoc
@DocMCotant The missing arm in idea — 3 month of oxaliplatin based treatment followed by 3 months of fluoropyrimidine for high risk pts — 5FU is the biggest positive differentiator — oxaliplatin is toxic sugar on top -
Less is more — demonstrated once again — neuropathy once established is a lifelong toxicity for patients to carry and this study demonstrates that we can give our patients back their lives without significant compromise -this study is very consequential as we have moved to the molecularly targeted era in advanced stages - limited induction chemo then let biology dominate the rest of the course @OncBrothers@OncoAlert@MayoCancerCare
This is one of my favorite papers we’ve published , led by our great Mayo resident @WallaceKleinS
“Reduced oxaliplatin exposure in gastrointestinal cancers: A
systematic review and meta-analysis”
We already know from maintenance trials in mCRC that stopping oxaliplatin after 3–4 months is not detrimental for PFS/OS and is associated with less neuropathy.
The question in 2026: what is the evidence in mCRC, the adjuvant setting, and other GI cancers? Is there a situation where oxaliplatin is still needed beyond 3–4 months of induction? In other words — is reduced oxaliplatin exposure associated with worse DFS or PFS across GI cancers?
We did a meta-analysis across GI cancers and settings:
• No statistically significant survival difference overall
• Lower neuropathy and grade ≥3 adverse events
Take-home: don’t keep oxaliplatin going beyond 3–4 months. These findings support limiting exposure to improve the therapeutic index of oxaliplatin-based chemo across GI settings.

Excellent work, practical and immediately applicable to my (and maybe your) clinic. And I got to meet @WallaceKleinS this week which made it even better...
Inaugural Debate Theme Reveal: Texas GI Tank 🦈 One of the biggest questions in GI oncology is heading into the Tank: 1L HER2+ Gastric/GEJ: The Role of Immunotherapy & Patient Selection. Four sharks are ready to do some serious clinical due diligence: @YJanjigianMD, Scott Paulson, @GIcancerDoc, and Ryan Corcoran. They'll challenge assumptions, scrutinize the data, and decide which strategy is worth investing in. Because in the Texas GI Tank, evidence is the currency, data drives the valuation, and only the strongest pitch earns the sharks' buy-in. Who's making the pitch? Who's taking which side? Stay tuned.
