@DrSteveTaylor The paper states causality cannot be inferred so for them to also use a title in the paper that says the vaccine reduces risk is highly misleading. Why is this not an observation that people who opt to have the vaccine are less likely to have prodromal cognitive impairment?

@MichaelOkun DIS and DIT are still the cornerstone because they paint the typical clinical picture of RRMS also. Theres no consensus on what to do when someone can be diagnosed as biologically having MS but no typical clinical picture so there is a lot of caution about this
@ankitkant Resources are not equal across all hospitals. Ours has no mechanism by which blood results are sent to the requesting clinician. Not a problem for primary care to fix of course but mad how much risk we all acknowledge in health care

The issue, like all specialties, is about how to avoid diluting training for both GIM and neurology. And also balancing the need for outpatient demand which has rocketed
@jfdwolff It's very common to wait that long and somwtimes its not feasible in confused patients. Also if high suspicion of HSV could consider full treatment even with negative pcr. A lot here rides on the renal failure -recognition and treatment, for which details are lacking
