Very much hope everyone is having a great start to the weekend. Here is my weekly AI and digital oncology innovation post. Amazing advances are being made!
101 experts from 25 countries | New international consensus published in Urologic Oncology.
I am pleased to share our recent publication in Urologic Oncology Seminars and Original Investigations :
“Project international Delphi consensus recommendations for perioperative management of inguinal lymphadenectomy in cancer patients”.
Developed within the Penile Cancer Optimization Initiative (PeCOI), @GTumors and @CAU_URO , this study addresses the prevention and management of perioperative complications following inguinal lymphadenectomy, an area where considerable variability in clinical practice remains.
Key findings:
* 101 experts from 25 countries, representing a broad international and multidisciplinary panel.
* 86 clinical statements evaluated through a two-round modified Delphi process.
* Consensus achieved on 51 statements (59.3%), covering preoperative optimization, lymphatic complications, drain management, antibiotic and thromboembolic prophylaxis, wound complications, and functional rehabilitation.
* Identification of areas of clinical uncertainty requiring further research and prospective validation.
This study provides practical, consensus-based recommendations to support the standardization of perioperative care in inguinal lymphadenectomy, particularly relevant in penile cancer, where high-quality evidence remains limited.
It has been a privilege to work alongside such an outstanding group of internationally recognized experts, @LuisMedinaMD, @drmarcostobias , @jteoh_hk , Amandeep Arora , @drgaganprakash , @DoctorSotelo , and @SpiessPhilippe , as well as all the specialists who contributed to this international consensus.
My sincere thanks to everyone involved, with special thanks to Philippe Spiess, Marcos Tobias-Machado, @ARRODRIGUEZMD , and Luis G. Medina for their support and trust throughout this project.

We’re excited to announce our new partnership with the Global Society of Rare Genitourinary Tumors (GSRGT)!
Dr. Philippe Spiess, President and Founder of GSRGT, shared his excitement for this new partnership “... how the GSRGT can expand his outreach in a collaboration with the Urology Times for whom I have had a high regard for many years in the great quality work and impact you have on the urology community and our patients. It would be an honor to explore these next steps together.”
We’re grateful for the kind words and excited for what we’ll accomplish together in support of the urology community.

🚨 Divergent Differentiation & Histologic Subtypes in UTUC
👥 3,441 patients from 22 institutions undergoing radical nephroureterectomy.
🔬 Divergent differentiation showed outcomes comparable to stage-matched pure urothelial carcinoma, while histological subtypes were associated with particularly unfavorable outcomes, emphasizing the importance of pathological charecterization. 🎯
Congratulations to the authors for multi-institutional collaborative effort!

🧬 Are we testing broadly enough for inherited cancer risk in urinary tract cancers?
A new study reports pathogenic/likely pathogenic germline variants in ~9% of patients with urinary tract cancers.
What stands out even more:
⚠️ More than one-third of carriers would have been missed by current guideline-based testing criteria.
That raises an important question for daily practice:
Should germline testing become broader and less dependent on traditional clinical selection criteria in GU cancers?
🎯 Clinical takeaway: if current criteria miss a substantial proportion of carriers, we may need to rethink who gets tested — not only for the patient, but also for relatives who may carry the same inherited risk.
Congratulations to the authors for this important work 👏
