Outcomes of pts with MIBC who decline RC in perioperative setting:
Take-home message:
1. cCR (Milan criteria) is a reliable biomarker and indicator of outcomes with Systemic Rx and reTURBT
2. Pts who do not get cCR could have a very poor prognosis under bladder-saving approaches > intensification with RC/chemoRT is very much needed
Looking forward to ongoing trials (eg EV209, EV309). Congrats to the entire team, also getting Best Poster! @OncoAlert@myESMO@CMercinelli@Anto_cigliola@BrigidaMaiorano@urotoday Piacentini Nasso Serafin et al.
Outcomes of patients with muscle-invasive bladder cancer who declined radical cystectomy in perioperative therapy trials conducted at San Raffaele Comprehensive Cancer Center - ESMO Open https://t.co/VhOHVA1GWH
With #GDFatherNeo#ESMO26 We brought visugromab+nivo combination neoadjuvantly in pts with MIBC after the initial phase 1 data in refractory setting.
33% CR rates vs 8% Nivo+placebo
All pCR pts still event-free (median FUP 15.7m)
Only 1 relapse event in major responders in N+V arm (pTa relapse)
4/5 pCR skipped RC in N+V arm.
Safety was quite good.
Pre-existing CD8+ infiltration associated with N+V Response.
It was an illuminating academic-industry collaboration for me.

What do we gain by adding belzutifan to pembro in high-risk ccRCC post-nephrectomy?
Our reconstructed IPD analysis of LITESPARK-022 pembro+belzu arm vs KN-564 placebo arm, now in @ESMOOpen, reveals a compelling survival signal:
🔹 DFS: HR 0.55
🔹 OS: HR 0.56

🚨 Is gemcitabine-cisplatin still the benchmark in MIBC?

Defining the Optimal Therapeutic Approach in Muscle-Invasive Bladder Cancer: A Systematic Review and Meta-Analysis of Novel Strategies Versus Gemcitabine-Cisplatin
A meta-analysis of three phase III trials in cisplatin 🧪eligible muscle-invasive #BladderCancer shows that intensified or immune-based neoadjuvant regimens outperform gemcitabine-cisplatin. ✅EVP produced the strongest improvements in event-free survival, overall survival, and pathologic complete response, while GC plus durvalumab and ddMVAC also improved outcomes. ➡️However, cross-trial differences and limited follow-up warrant caution. The findings underscore a rapidly evolving perioperative landscape and raise important questions about biomarkers 🎯 , overtreatment, toxicity, and bladder preservation.
