📉 52% reduction in risk of radiographic progression or death (HR 0.48, P<0.001). 3-year rPFS: 77% vs 56%.
🎯 Benefit was strongest in BRCA1/2-altered disease (HR 0.37), but remained significant in non-BRCA HRR alterations (HR 0.57).
⏳ Fewer patients required subsequent therapy (HR 0.51) and PSA progression was delayed (HR 0.51).
❤️🩹 OS trend favored talazoparib (HR 0.77), but data remain immature.
🩸 Toxicity remains the tradeoff: Grade ≥3 anemia in 51%, transfusions in 40%, and 2 treatment-related deaths.

Can we safely stop ADT + ARPI in selected mHSPC patients after a deep response? 👀
The phase 2 A-DREAM trial suggests that some patients may enjoy prolonged treatment-free intervals with testosterone recovery after stopping therapy.
💡 Key findings from Abstract 5004:
▪️78 pts underwent treatment interruption
▪️58% remained treatment-free at 18 months
▪️41% remained treatment-free with testosterone recovery
▪️67% recovered eugonadal testosterone levels
▪️Median follow-up: 21.2 months
⚠️ Important caveats:
▪️Single-arm phase 2 study
▪️Highly selected exceptional responders
▪️No randomized comparator
▪️Long-term survival impact unknown
Still, this opens an important conversation around:
“Do all responders need indefinite continuous therapy?”
A major quality-of-life and survivorship question in modern prostate cancer care.

We at @MSK_RadOnc have already adopted a new cord constraint of D0.1cc < 19.1 Gy in 3 fractions, which is associated with a 1% risk of G3-4 RM 2 years after spine SBRT. This constraint applies to the true cord, or a cord planning organ at risk volume (PRV).
