🚨 Three phase III trials with perioperative chemoimmunotherapy. Different outcomes in MIBC.

NIAGARA: durvalumab + GC → EFS HR 0.68, OS HR 0.75 ✅

KEYNOTE-866: pembrolizumab + GC → EFS HR 0.88, OS HR 1.00

CA017-078: nivolumab + GC → EFS HR 0.94

Interestingly, all three showed numerically higher pCR rates, but only NIAGARA demonstrated statistically significant survival benefits in the reported analyses.

Why? Differences in trial populations, treatment schedules, statistical design—or tumor biology?

These are cross-trial observations, not head-to-head comparisons. We should resist attributing the differences simply to PD-1 versus PD-L1 inhibition.

The key challenge: identifying which MIBC patients truly benefit from adding immunotherapy to chemotherapy.

We urgently need predictive biomarkers and molecularly informed perioperative treatment selection. 🔬

🇪🇸 TALAPRO-3: ¿Un nuevo paso para los PARP inhibitors en prostate cancer hormonosensible?

Dr. Joaquín Mateo (@quimmateo) from @VHIO in Barcelona breaks down the TALAPRO-3 trial, presented at ASCO 2026 and published in the New England Journal of Medicine, alongside @AzadOncology, @neerajaiims and colleagues.

🧬 What did TALAPRO-3 show?

This phase 3 trial evaluated talazoparib + enzalutamide versus placebo + enzalutamide in patients with HRR-altered metastatic hormone-sensitive prostate cancer (mHSPC).

🔹 599 patients enrolled across both treatment arms.

🔹 Radiographic progression-free survival (rPFS) at 3 years: 77% vs. 56% (HR 0.48; 95% CI 0.36–0.65; p<0.001).

🔹 Overall survival (OS): 78% vs. 72% at 3 years in the interim analysis (HR 0.77; 95% CI 0.56–1.04). OS data remain immature.

🔹 Safety matters: Grade ≥3 anemia occurred in 51% of patients receiving talazoparib.

🚨 Important biomarker signal from #ESMO2026—highly relevant beyond breast cancer!

In the neoadjuvant TALENT trial T-DXd achieved relatively low pCR/near-pCR rates (16–17%) in HER2-low breast cancer.

Importantly, higher baseline HER2 expression was associated with better pathological response, while HER2 expression declined substantially during treatment.

Could the same principle apply to perioperative enfortumab vedotin + pembrolizumab in MIBC?

We urgently need to understand whether low or absent membranous NECTIN4 expression identifies patients less likely to achieve pCR—and potentially at higher risk of recurrence despite EV+pembro.

Histology, quantitative membranous NECTIN4 expression, spatial heterogeneity and ctDNA dynamics should be integrated into prospective biomarker studies. @Markuseckstein3 @DrRosenbergMSK @Dr_Aggen

Perioperative EVP for MIBC approved in Europe by the EMA irrespective of platinum eligibility. This is based on B15/KN905 with ⬆️ ⬆️ pCR, EFS & OS vs previous standard of care. More patients will get systemic therapy (cisplatin obstacles won’t exist) and these patients will have better outcomes. Will VOLGA tell us more about the optimal number of cycles #ESMO26 ? Which patients don’t need surgery (EV209, EV309, TROFUSE55). Will ctDNA help us?

Telix has announced a collaboration with Ekalia, a joint venture between IBA and Framatome, to advance scalable U.S. production of astatine-211 (²¹¹At), a promising radioisotope for targeted alpha therapy.

The collaboration will establish a dedicated U.S. source of ²¹¹At, with the first specialized alpha-beam cyclotron to be installed at our RLS Radiopharmacies site near Philadelphia, Pennsylvania.

This will support Telix’s investigational alpha therapy pipeline, provide isotope access for third-party academic and industry programs, and further strengthen our vertically integrated radiopharmaceutical supply chain.
