Will we see more integration between modern medicines and traditional systems?
THE EVOLUTION OF MEDICINE
What if the next revolution in medicine is not a choice between 🌿nature and the laboratory?
What if it is a reunion of knowledge systems we spent centuries separating:
➡️Ecology + Chemistry
➡️Traditional Knowledge + Clinical Trials
➡️Botanical Diversity + Precision Medicine
For most of human history, pharmacology began with the living world: plants, fungi, minerals, observation and knowledge passed between generations.
Consider🌱Cannabis: a modern integrative medicinal approach to healing can include cannabinoids.
In 1640 John Parkinson - "Apothecary of London, and the King's Herbalist" - published 'Theatrum Botanicum' under the King's "especiall privilege."

🌱Cannabis/hemp appears in the work; historical medical uses included preparations of the root for gout, inflammation and joint pain.
In 1652, Nicholas Culpeper's "English Physitian" also included Cannabis/hemp.
🪩But history gets more interesting when medicine meets commerce.
In 1687 the Scottish Privy Council granted Thomas Weir rights to prepare and sell Dr Anderson's famous 'Scots Pills' which contained aloes, colocynth and gamboge.
In 1698 physician Nehemiah Grew received a 14-year English patent for manufacturing medicinal salts.
▶️Then America built its own IP system.
The first U.S. patent statute arrived in 1790. The first patent - signed by George Washington went to Samuel Hopkins for a method of making potash.
Roger Adams (1889-1971) was one of the leading American organic chemists of his generation. Adams isolated and identified cannabidiol (CBD) from Cannabis sativa and investigated its relationship to tetrahydrocannabinol (THC). In 1940 his clinical group published a study specifically describing the isomerisation of cannbidiol to tetrahydrocannabinol.
Adams filed a U.S. patent 2,304,669 which was granted in December 1942. The patent described a chemical process for obtaining pure cannabidiol.
He filed further patents - US 2,419,936 in 1942 and US 2,419,937 in 1944.
However, although Adam's work was foundational, it should not be described as the final identification of THC. Yechiel Gaoni and Raphael Mechoulam's landmark 1964 work isolated and established the structure of the principal active THC and took the Cannabis story into modern cannabinoid pharmacology.
▶️The 1800s became a turning point.
Opium increasingly became morphine and other isolated alkaloids. Coca yielded cocaine. Foxglove pharmacology moved toward purified cardiac glycosides.

▶️By the turn of the 20th century, another transformation was underway: industrial pharmaceuticals.
Bayer launched Aspirin commercially in 1899; tablets followed in 1900.
Synthetic organic chemistry also produced Salvarsan, introduced in 1910 as an effective treatment for syphilis.
▶️Then came 1910 and the FLEXNER REPORT.

Abraham Flexner surveyed all 155 medical schools then operating in the U.S. and Canada. Within 15 years, 89 of those schools had been closed.
Five out of the seven medical schools devoted to educating coloured physicians closed.
Women's access also narrowed, and the wider reform movement targeted many schools associated with what Flexner called medical "sects."
▶️The U.S. Marihuana Tax Act was enacted on 2 August 1937 imposing federal registration and taxation controls on 🌱Cannabis.
Here's the paradox: modern medicine never really left plants.

Artemisinin - artemesia annua
Aspirin - the salicylate lineage of willow/meadowsweet

▶️Now something remarkable is happening:
WHO's Global Traditional Medicine Strategy 2025-2024 calls for evidence-based integration of traditional medicine with stronger research, regulation, safety and health-system integration.
The U.S. National Cancer Institute holds 230,000+ natural product extracts and is generating 1M+ research-ready samples for modern screening.

▶️It is a new frontier for pharma.
But the next era must do something the previous one too often failed to do:
Remember where the knowledge came from.
The 2024 WIPO treaty on generic resources and associated traditional knowledge points in the same direction - toward disclosure of the origins/providers of knowledge used in patent inventions.
