WHAT DID WE DO?
We used high-spec functional magnetic resonance brain imaging (fMRI), and we detected and compared brain metabolites in 55 adults with type 1 diabetes (including painful diabetic peripheral neuropathy (DPN), painless DPN, without DPN) with 20 healthy controls. Proton magnetic resonance spectroscopy measurements (N-acetylaspartate (NAA), glutamate (glu), myo-inositol (mI), and glycerophosphocholine (GPC) were obtained in ratios to creatine (cre)) from the parietal region, anterior cingulate cortex and thalamus.
AND WHAT DID WE LEARN?
Nerdy data😉:
The diabetes group revealed decreased parietal NAA/cre compared to healthy controls (1.41 ± 0.12 vs. 1.55 ± 0.13,p < 0.001) and increased mI/cre (parietal: 0.62 ± 0.08 vs. 0.57 ± 0.07,p = 0.025, cingulate: 0.65 ± 0.08 vs. 0.60 ± 0.08,p = 0.033). Reduced NAA/cre was associated with more severe DPN (all p ≤ 0.04) whereas increased mI/cre was associated with higher hemoglobin A1c (HbA1c) (p = 0.02).
Conclusion🎯:
We found, that specific metabolic brain profiles are linked to the different phenotypes of diabetes, DPN and painful DPN.
AND WHY IS THIS IMPORTANT?
Neuropathy is generally poorly understood. We suggest, that the brain is much more interwoven in symptoms and patho-physiology and all many other aspects, than hitherto acknowledged. This study delivers building stones and some evidence, that the brain plays a role in more distal neurological deficits, indicating the importance of the 3rd neurone👍

About neuropathy:
Nerve damage in diabetes affects up to half of all people with diabetes, and it may affect any part of the nervous system. It is without symptoms to begin with and therefore often goes unnoticed by people with diabetes. And some people may be seriously affected, while others go free. The reason why, remains an enigma. Of all consequences, the #Diabetic #Foot #Ulcer, is the most serious, a direct result of the reduced or often completely absent sensation in the feet - and this leads to #Amputations in far far to many people, incurring immense human sufferings, high costs to society and huge burdens for our healtcare system.
And adding to this, a great many people with diabetes may experience painful symptoms in their feet and legs, even if they have no sensation in their feet. These symptoms range from slight and bearable to excruciating and incapacitating, and there is no efficient treatment and we do not understand the pathophysiology.
About the #MEDONproject:
These are some of the reasons, the MEDONproject Research Group is chasing robust clinical neuro endpoints and measurements, that are so much needed for research and clinical work, and the development of efficient drugs.
We are doing so in tightly knit collaborations between clinical departments (i.e.: Dept of Endocrinology, Dept of Radiology, Steno Diabetes Center North Denmark, Aalborg, Denmark) and basic science (i.e.: Integrative Neuroscience and Center for Neuroplasticity and Pain at Aalborg University, Danish Pain Research Center Aarhus). In essence, we couple clinical studies with the most advanced cutting edge neuro-technologies available - ranging from the cell membrane level, nerve fibre examinations at a cell level, deep clinical phenotyping over robust clinical studies to transformation and translation into daily clinical and research practice.
Our overall aim is to develop specific high-tech methods, that will enable us to detect the earliest signs of nerve damage in the individual person with diabetes, allowing early prevention and optimal treatment, to preserve sensation in the feet and diminish foot ulcers, amputations and painful neuropathy
