This prospective observational study investigated #ProstateCancer (PCa) incidence and characteristics in 225 male carriers of pathogenic germline variants in mismatch repair (MMR) genes, specifically focusing on the MSH2 and MSH6 genes, who underwent annual PSA screening in Norway. Of the participants, 22 (9.8%) developed PCa, with MSH2 and MSH6 carriers showing the highest incidence rates (14.5% and 13.1%, respectively). Half of the tumors had a Gleason score ≥4+3, indicating aggressive disease, and a substantial proportion had advanced pathological stages. The standardized incidence ratio (SIR) for all MMR carriers was 9.54, with even higher SIRs for MSH2 (13.0) and MSH6 (13.74) carriers, suggesting a significantly increased PCa risk.
These findings support the continuation of PSA screening for MSH2 and MSH6 carriers, though further studies are needed to refine screening recommendations for other MMR gene carriers.
