This is one of the hardest and most personal things I have ever shared.
His name was William, but his family and friends called him Billy. To me, he was Uncle Billy.
Uncle Billy was terminally ill and living with pain around the clock. He did not have access to the pain medication or comfort care he needed. In the final note he left our family, he wrote, “Can not continue like this,” and described facing the end of his life alone in a hospital bed in “24/7 pain. No cure.”
He later died by suicide.
Those words have never left me. He knew he was terminal. He was not asking anyone to cure him. He was asking not to suffer while he was dying.
I am not sharing his final note for attention or shock. I am sharing it because Uncle Billy was a real person who was deeply loved. His life mattered. His pain mattered. His last words deserve to be heard.
His story shows the dark side of what can happen when someone in unbearable pain is left without the relief and comfort every human being should receive.
This is one of the main reasons I fight so hard for pain care. I fight in Uncle Billy’s memory, for myself, and for every person living with chronic or terminal pain who has been dismissed, judged, or treated like a problem simply because they needed relief.
The opioid crisis caused real harm, but so have the fear, stigma, and one-size-fits-all thinking that followed it. When fear of medication becomes more important than the suffering person in the bed, something has gone terribly wrong.
Responsible prescribing is not the same as reckless prescribing. Needing medically appropriate pain medicine does not automatically make someone an addict. People with serious pain deserve to be evaluated as individuals, and doctors should be able to use their medical judgment without fear replacing compassion.
Uncle Billy deserved comfort. He deserved dignity. He deserved to know he would not be abandoned to unbearable pain at the end of his life.
No terminally ill person should have to spend their final days suffering this way. No person living with chronic pain should have to beg to be believed. No family should be left wondering whether compassionate care could have changed the final days of someone they loved.
Please share Uncle Billy’s story in his memory and for every person suffering behind closed doors. Let his final words show the human cost of untreated pain and remind people why compassionate pain care still matters.
Uncle Billy, I am sorry you suffered. I am sorry the relief you needed never came. I love you, I miss you, and I will keep fighting in your memory."
