Anti-#Ro52 and anti-#Ro60 profiles define distinct #Sjögren’s disease (SjD) subsets, as demonstrated by a retrospective study of 725 primary SjD patients stratified by anti-Ro52/Ro60 status: ◦ Distribution: 50.6% double-positive (Ro52⁺Ro60⁺), 17.7% Ro52⁺Ro60⁻, 10.2% Ro52⁻Ro60⁺, 21.5% double-negative. ◦ Anti-Ro52 positivity (Ro52⁺Ro60⁻ and Ro52⁺Ro60⁺) → significantly higher ESSDAI scores (greater disease activity). ◦ Double-positive (Ro52⁺Ro60⁺) independently associated with: - Renal involvement (OR 5.53) - Cutaneous involvement (OR 2.49) - Hyperglobulinemia (OR 5.19) - Low complement (OR 1.70) ◦ Isolated Ro52⁺ (Ro52⁺Ro60⁻) specifically linked to interstitial lung disease (OR 4.17). ◦ Significant synergistic interaction between anti-Ro52 and anti-Ro60 for hyperglobulinemia. These findings indicate that anti-Ro52 positivity marks higher severity, and double positivity identifies a multi-organ, hyperactive immune phenotype.
