#SNOPs Frailty at d front door team turned around 23 patients in d last 4 days. So nice to see 13 patients total in ED at 0730 today despite many beds closed throughout. #Frailtyfirst works! 🥰 @DanielleSchone1 @emma091_emma #therapies EDteam @HayleyKendall13 @wuthnhs

#SNOPs providing person centred care.@JoanwrightNHS did bank shift yesterday, turned 5 patients around back home. Made sure they were fed, well looked after. Also pushed pts for X-rays and got them into family’s car to get pt home as too late for patient transport!! @wuthnhs

The #SNOPs try to see them as they arrive together with an allocated ED SHO - aiming for early analgesia, diagnostics and therapy assessment early. All with a mindset of Triple assessment and using ambulatory (OPRA clinic) or #CIRT services as alternatives to admission

We discussed how we can stretch beyond the norms of practice in our effort with earlier triage/ assessment / investigations and planning for home. Joint assessments btw medical and therapy team or our #SNOPs with @sparky2409791 #rapidresponse.
