Do real-world data confirm trial data of the new SOC in first line?
The rationale for treating a 79-year-old female patient with muscle-invasive bladder cancer, based on exclusion criteria from the EV-302 trial, highlights the importance of clinical eligibility for enfortumab vedotin (EV) and pembrolizumab therapy. Patients must be fit for cisplatin- or carboplatin-based chemotherapy, with an ECOG performance status of 0-2 and no severe heart failure (NYHA III), uncontrolled diabetes, or significant neuropathy. The UNITE study suggests that EV/pembrolizumab offers strong disease control outside clinical trials, though overall response rates (ORR) may be lower. Biomarkers like TP53 and KMT2D mutations could indicate poorer outcomes. Data from the Austrian registry also show that patients with high comorbidities (CCI >5) tend to have worse outcomes due to reduced therapy tolerance. Real-world data on EV/pembrolizumab remains limited, emphasizing the need for further studies.
