After MVR, on second post op day surgeon heard a pansystolic murmur, cardiologist is asked to rule out paravalvular MR. Patient stable hemodynamically, extubated, comfortable. Loud PSM is heard, echo shows acquired LV- RA shunt. This is already reported, see a case report.
