Day 5 #indapamide ADEs: Common incl hypersensitivity reactions, rash & clinically significant electrolyte disturbances e.g hypokalaemia; needs pre-therapy & regular monitoring. Rare/serious incl angioedema,renal failure, SCARs, aplastic anaemia, vision disorders (NOT exhaustive)

Day 4 MOA: #indapamide ↓ BP mainly via vascular action (not diuresis) by Ca+ uptake impairment > ↓ smooth muscle cell reactivity. Also vasodilation via ↑ PGE2, prostacyclin & bradykinin levels (also relevant to ↓ platelet aggregation). Reduces left ventricular hypertrophy.

Day 3: Kinetics #indapamide;good oral absorption, cmax 1-2 hrs. Binds to RBCs & plasma proteins, then taken up thru vascular walls where it acts. Liver metabolism via CYP3A4 with 1 active metabolite. Renal excretion (avoid in severe impairment), but ~ 23% GI excr. T½ 15-18 hrs

Day 1: Labelled a ‘thiazide-like’ diuretic (as acts in the distal convul tubule), #indapamide is a non-thiazide sulfonamide. Derived in late 60s from benzamide (found in plants e.g autumn crocus), it was designed to retain blood pressure lowering effect as separate from diuresis

Just rx’d #canagliflozin to a diabetic w eGFR 37ml/min and #indapamide to a frail older diabetic w uncontrolled HTN on HCTZ. Am I doing this right @ChristosArgyrop ?
