Back then doctors missed signs of fatigue, failed #carpaltunnel surgery, neuropathy, #macroglossia, shortness of breath, and skin/hair/nail changes. No one did an echo. VAD chemo was off-label but started too late to help. That was 2003. So much is different today. Today there is hope.

In this episode of All Things Amyloid, we hear from Dr. Mazen Hanna, cardiologist at the Cleveland Clinic and co-director of the Amyloid Program. Adapted from his video “Cardiac Amyloidosis - AL and ATTR: Two Different Conditions” he explains how cardiac amyloidosis can originate from two very different types of amyloidosis: AL (light chain) or ATTR (transthyretin). Dr. Hanna illustrates how clinicians can identify cardiac amyloidosis and look to different diagnostic work-ups to understand whether the issues are due to AL or ATTR amyloidosis. These two conditions are treated differently and have different prognoses, emphasizing the importance of identifying the type of amyloidosis involved. #Macroglossia is a potentially under-recognized feature of inclusion body #myositis (IBM). In case reports and published series, patients with IBM and macroglossia seem to correspond to a phenotype characterized by early bilateral facial weakness at presentation in tandem with dysphagia and bulbar impairment, followed by respiratory failure requiring non-invasive ventilation. These patients did not respond to steroids or IVIG. Tongue MRI can show muscle hypertrophy and fatty deposition, and muscle histology demonstrates supportive features of IBM in these cases.

She died 7 months after she was diagnosed.
If her hand surgeon had sent a tissue sample off to be biopsied for #amyloid proteins, she could have lived much longer.

If you are an orthopedic surgeon, or know one, this is a must read!! In a 2020/2021 study published in the Journal of Hand Surgery, of 62 patients screened 22.5% tested positive for amyloid. With drugs available now for treatment, the key is getting diagnosed.
