POCUS case of the day. Patient with pneumonia under mechanical ventilation using vasopressors. We measured CVP, which was 12. However, IVC showed 2 different dimensions depending of which view was insonated.

Lets talk about some extended views I like in the ED: Subcostal Short Axis!
Struggling with Parasternal views? No problem!
The Subcostal Short Axis is a lifesaver.
How to:
From Subcostal 4C, rotate probe 90° counterclockwise.
Index marker to head or 1 o'clock.
Adjust depth: 16-24cm to image the entire LV.
Optimise:
- Place the LV in the centre of the image (rock the probe).
- Make the LV appear round in shape (rotate probe)
- Visualise both papillary muscles attached to the LV wall (tilt probe)

BUT - what about when these two scans are seemingly contradictory? 😕
First - make sure your technique is on point 💯
- Always scan in two planes (short and long axis, especially the IVC!)
- Avoid common pitfalls like assessing the IVC right at the diaphragm - the latter can either pinch or stent open the IVC, confounding its evaluation
- Make sure you're assessing multiple parameters: IVC shape (probably the most important), size, and collapsibility
(Now, that's not to discount the potential confounder of large-volume ascites, which can of course falsely compress the IVC).
BUT the point remains in #POCUS: if you have the chance, always scan things in more than one plane! 🤓

Straw poll of half dozen POCUS learners yesterday, all had attended various POCUS courses in the last year. None had been taught how to assess IVC in short axis. This is a crucial skill! Is the focus on long axis just tradition?
