@RJmdphilly Yes. Another view: The problem is that wellness is framed as a problem a champion can solve. Champion versus complex, multi-dimentional, social-structural, individual-environment experience? Doesn't seem like the right solution for the problem. #solvechat

@projsolve #solvechat solutions are implemented and data is collected over a period of time, then evaluated for efficacy. For example, sepsis alerts were implemented and over time it was found that time to fluids/abx were improved.
@projsolve #solvechat Hmmm simply put, both articles are saying “be better.” A purest might say you’re sacrificing methodologic rigor, but I’m sure a balance can be struck as suggested in the Annals article.
@projsolve #solvechat I think the difficulty here tends to be how to apply solutions from elsewhere to your own situation/institution. A variety of small logistical differences between facilities compound to make solutions difficult to translate.

@projsolve #solvechat maybe not just a portal for QIs that have been done, but also just problems or thoughts - maybe someone has an idea but doesn't know what to do about it. A list of current issues and see if someone else can bring something new to the table.

time. Our discussions at the time centered around maybe having a resident "away clinic" day when you'd travel to particular areas for these things with a few basic supplies. Probably, studying readmission rates would come much later. #solvechat (forgot to tag before!)
#solvechat I can only speak from my intern year. We'd have a lot of patients who had multiple hospital visits due to inadequate access to healthcare when needed - usually wound/ access site checks, medication refills - small issues at first that became big when not addressed on
#solvechat I prefer the idea of protected time rather than financial incentives. Financial incentives may put way too much pressure, may lead to more "convenient" results, and lead to a greater frustration and burnout in case of negative results.
