How to differentiate between Edward and Patau.

Pa (प) se Patau.
If you hear the pa sound - it’s in Patau. If not - it’s Edward.

Holo‘p’rosencephaly, Polydactly, Polyscystic kidney, Cleft ‘P’alate, cutis a’P’lasia- all are in Patau.

You don’t hear ‘P’
Rocker bottom feet, horse shoe kidney, clenched fist with overcrossing finger - It’s Edward.

(Also- E=Eighteen- Edward is Trisomy 18, P= Patau= puberty- which starts at 13- Trisomy 13)

Erythema Nodosum ≠ Erythema Induratum

Both present with tender leg nodules - but the differences matter clinically and histologically.

• EN: anterior shins, non-ulcerating, septal panniculitis (no vasculitis)
• EI (Bazin): posterior calves, often ulcerates with scarring, lobular panniculitis with vasculitis, TB-associated

A simple distinction that prevents misdiagnosis.

Identify the Nerve. Remember the Pattern.

1️⃣ CN III (Oculomotor) – “Out & Open”
➊ Down & out eye
➋ Ptosis
➌ Dilated pupil

💡 Down & Out + Blown Pupil = CN III Palsy

2️⃣ CN IV (Trochlear) – “Tilt”
➊ Vertical diplopia
➋ Worse looking down/stairs
➌ Head tilt away from lesion

💡 Vertical Diplopia + Head Tilt = CN IV

3️⃣ CN VI (Abducens) – “Abduct”
➊ Cannot abduct eye
➋ Horizontal diplopia
➌ Esotropia

💡 Often affected by raised ICP

4️⃣ CN V (Trigeminal) – “V Face”
➊ Facial sensory loss
➋ Weak mastication
➌ Reduced corneal reflex

💡 Sensation + Chewing = CN V

5️⃣ CN VII (Facial) – “Face Falls”
➊ Facial weakness
➋ Loss of forehead wrinkling (LMN)
➌ Loss of taste (anterior 2/3)

💡 UMN = Forehead spared | LMN = Forehead involved

6️⃣ CN VIII (Vestibulocochlear) – “Vertigo & Volume”
➊ Hearing loss
➋ Vertigo
➌ Nystagmus

💡 Hearing Loss + Vertigo = CN VIII

7️⃣ CN IX/X (Glossopharyngeal & Vagus) – “Gag Gone”
➊ Dysphagia
➋ Hoarseness
➌ Reduced gag reflex

💡 Hoarseness + Dysphagia = CN X

8️⃣ CN XII (Hypoglossal) – “Half Tongue”
➊ Tongue deviates toward lesion
➋ Wasting & fasciculations
➌ Dysarthria

💡 The tongue licks the lesion.

It does not mention the 2.7 million acres of Vietnamese forest doused in Agent Orange, a chemical weapon disguised as herbicide, that is still deforming Vietnamese children today.

Not in 1970. Not in 1985. Today.

Children born in 2020 with bodies twisted by a war their grandparents fought.

And the chemical companies that made it are still in business.

Still profitable.

Still un-prosecuted.

And yet they send us human rights reports.

They grade our democracy.

They warn us about our behavior.

The audacity is so enormous it becomes almost impressive.
