A Paradigm Shift in Osteoarthritis Treatment?

This may be the most consequential osteoarthritis study in over a decade.

But it hasn’t been talked about.

A landmark 10-year randomized controlled trial from Russia, discussed personally with the authors by myself and Dr. Richard Shaffer, has shown that low-dose orthovoltage radiotherapy (LDRT) may do more than relieve symptoms:

It may actually slow or halt structural progression of knee OA.

The results are staggering:
🌠 Only 3% of patients treated with LDRT progressed ≥2 KL grades over 10 years, compared to 36.1% in the control group.
🌠 Synovial inflammation, a known risk factor for progression, had no effect in those receiving LDRT, suggesting a powerful anti-inflammatory and disease-modifying mechanism.

This isn't just pain relief.

This is joint preservation.

If validated, these findings could alter the treatment landscape, not just for radiation oncologists, but for orthopedic surgeons and rheumatologists as well. LDRT could become the first therapy capable of changing the natural history of osteoarthritis.

But let’s temper our enthusiasm:
This is a single-institution study. The excitement is real, but so is the need for replication in large-scale, multi-center trials.

Duration of Androgen Suppression with Postoperative Radiotherapy (DADSPORT) for Nonmetastatic Prostate Cancer: A Collaborative Systematic Review and Meta-analysis of Aggregate Data

This systematic review and meta-analysis by the DADSPORT Collaboration evaluated the impact of hormone therapy (HT) added to postoperative radiotherapy (RT) in nonmetastatic #ProstateCancer across five randomized controlled trials involving 4,411 participants. While HT did not significantly improve overall survival (OS) in the general population, it did show meaningful benefits in metastasis-free survival (MFS) and prostate cancer–specific survival (PCSS), each with a 4% absolute improvement at 8 years.

Notably, OS benefits may be limited to patients with higher pre-RT PSA levels or CAPRA-S scores.

These findings support the use of adjuvant HT to improve cancer outcomes, particularly in higher-risk patients.

🧬 ENRT+MDT:
✅ Superior bRFS (57% vs 41%, p=0.014)
✅ Superior pelvic control (lrRFS 85% vs 62%, p=0.006)
✅ Superior MFS (76% vs 63%, p=0.06)
❌ No increase in ≥G2 GU/GI toxicity
