Pharma’s direct-to-consumer play through TrumpRx probably won’t move the needle on net drug costs. It’s less about savings and more a direct strike at PBMs and their rebate games.
Any “discount” patients see is basically the rebate that the PBM would have received.

@chrissyfarr It’s not less than 1%. Getting systems to invest people time is far harder than getting them to pay money. If a startup makes it through the sales gauntlet, it means it’s solving a problem the org really needs to solve. At least 1 champions will fight to see the pilot stay alive.
@john_chilmark That’s a popular narrative and one that fit the description of most vendors. The better vendors know the ideal workflows, but the clinic/hospital decides to maintain the status quo despite being urged otherwise by the vendor.
The latest post by @waitbutwhy calls this type of culture an Idea Lab. It’s the best mental model I’ve seen to describe the dynamics of a high performing start-up team. Great teams should strive to become Idea Labs.

@jasonfried@PeterAttiaMD Hi Jason - can you talk about employee incentive structures and how they can create “calm” workplaces? I know at Basecamp you have foregone traditional options grants in favor of profit shares/an employee pool if Basecamp were to exit. Has that worked for you?
