Yes, the NHS is complex, and “simple fixes” often become tomorrow’s embedded nonsense.

But defending the current model is also scarcity thinking.

We have built a system of rationed cognition: scarce appointments, scarce triage, scarce diagnostics, scarce senior judgement, then we act surprised when queues breed queues.

Post-scarcity healthcare means designing for abundant intelligence and capacity, not moving the bottleneck from GP reception to the pharmacy counter.

Exactly this. The NHS is still behaving as if expertise must remain scarce, rationed through limited training places, bottlenecked supervision and slow workforce planning.

But radiology is a perfect case for post-scarcity thinking: train more humans, wrap them in AI support, use senior expertise better, and stop treating cognitive capacity as a fixed Victorian asset.

The future is not fewer radiologists. It is more radiologists, amplified by abundant diagnostic intelligence.
