A new study answers a key clinical question:

👉 How accurate is TTE in detecting outflow graft abnormalities?
✨ Key findings:
🎯 Specificity = 100% → if you see it, it’s real
⚠️ Sensitivity = 61% → but you may miss cases
📍 Much better for:
👉 Distal graft (71% sensitivity)
❗ Limited for:
👉 Proximal graft (only 33%)
📊 As shown in the graphical abstract (page 2):
👉 TTE can identify:
🔁 Kinking (≥90° bend)
🚫 Stenosis (≥50%)
👉 Doppler clue:
⚡ Vmax >2.0 m/s suggests obstruction
💡 Clinical message:

🧠 Great for screening & follow-up
🧪 But CT remains gold standard for diagnosis

🔷️Hasta 20-45% de los pacientes sometidos a TRC no obtienen una respuesta clínica significativa, a pesar de cumplir los criterios clásicos de selección basados en el QRS y la FEVI (un QRS ancho no siempre significa disincronía mecánica). 🤔⚡️🫀
🚨Los dos hallazgos ecocardiográficos clave son: ⤵️
🔹 Destello septal [Septal Flash, SF]: movimiento rápido y precoz del septo hacia el VI inmediatamente después del inicio del QRS, frecuentemente seguido de un rebote. 💢💞
🔹 Balanceo apical [Apical Rocking, ApRock]: movimiento bifásico del ápex causado por la contracción secuencial del septo y posteriormente de la pared lateral.
✅️La presencia de cualquiera de estos 2 signos confirma la existencia de disincronía mecánica y se ha asociado con una mayor probabilidad de remodelado inverso, mejor respuesta clínica y mejor supervivencia tras la TRC.📈⚡️
