KEYNOTE-937 is a letdown, but the story feels familiar 😕🩸
Phase 3 KEYNOTE-937 tested pembrolizumab after surgical resection or local ablation for HCC. This was the long-awaited attempt to bring IO into the adjuvant setting after cure-intent therapy.

Median RFS 46 vs 45 months
4-year RFS rate 50 vs 50 percent
No separation. No late tail. No signal.
🧠 Historical context matters
We’ve now seen this movie before. IMBRAVE-050 also failed to meaningfully move the needle in the adjuvant HCC space. As frustrating as this is, it’s becoming clear that simply dropping IO after surgery is not enough.
🧐 Takeaway
Disappointing, but not totally unexpected. The biology is telling us something. If we want to change outcomes in HCC, the action is likely earlier.
It’s time to double down on neoadjuvant and perioperative strategies, where immune priming and tumor antigen exposure may actually matter.

FDA just approved peri-op durvalumab+ FLOT for resectable gastric/GEJ cancer based on MATTERHORN 👏 Huge wins for patients, especially PD-L1+ (which was ~90% of the trial). Congrats to investigators

Back on Twitter because @Aetna is denying HIPEC for my patient with peritoneal metastases from colorectal cancer - a well established standard of care in the United States for fit patients with reasonable PCI. I cannot even reach a physician to speak with.
As someone gravely ill with a brutal #cancer I want to say this to my fellow Americans: You only have one life. I urge you to spend it loving and caring, not hating and filled with grievance. Every day we have the chance to start over, to be better, to lift others up. Do that.
