In 2024, the U.S. spent $250B on specialty drugs, across biologics, gene therapies, and complex cancer treatments. By 2033, that figure is expected to exceed $1.5T annually. Yet the infrastructure that governs access to these drugs still runs on faxes, phone calls, and broken web portals. Care is delayed, patients wait, and lives hang in balance.

Mandolin is building a new operating system for access, powered by AI agents that act like the best back-office employee — but faster, works 24/7, and rarely makes mistakes. Our agents interpret medical policy in real time, chase missing documentation, and push prior authorizations, claims, appeals through brittle infrastructure — turning weeks into minutes, and eliminating red tape.
