In November 2023 Marieha Hussain went on a peace in Palestine march with a clearly satirical placard depicting the PM and then Home Secretary as coconuts. The placard was a mild intra-communal version of the political satire you’d see in British newspapers on any given day. 1) they force, not convince.
2) multiple trials have shown physician uptake is <50% even in audits, the "stick" approach fails, but is all that is offered.

As some one who has worked in the NHS for 35 yrs and examined its working throughout that time my list of what needs changing would start with

Stop breaking the system by defunding it in the name of efficiency. It is way beyond the point at which trying to cut things can improve efficiency. It is doing the opposite. Now not enough diagnostic centres, not enough doctors, not enough beds (numerous reports - ask @NuffieldTrust or @TheKingsFund or @HealthFdn)

The rest are relative minutiae but areas to address include

* understanding that promotion of the private sector as a realistic alternative to the NHS is flawed. The private sector can do sone things well (low risk procedural
stuff) but is no substitute for real NHS hospitals with the full gamut of services (staff, systems, space)

*don’t dumb down medicine.
There appears to be a concerted effort to suggest that medicine has become less complex, easier to learn, answers available on google etc and as a consequence medicine can be taught over a shorter period and the role substituted by others who are notably less trained/qualified.
The truth is that medicine has become more complex, delivered to ever older more complex and more comorbid patients with ever higher expectations (most of which are actually met). https://t.co/MBvAISjraq This all happens in the framework of an environment in which litigation (certainly costs and numbers to an extent) is mushrooming. https://t.co/96MGGYPbjc While there may be value in working in new ways the lie that doctors can be replaced by less qualified alternatives because the job has become easier is misplaced, disingenuous and dangerous (as well as quite likely being cost-ineffective). There is a danger these systems will be brought in without testing of efficacy, safety or cost effectiveness.

I have just finished another set of incredibly difficult weekend nights as the A&E consultant incharge.

Some thoughts:

The NHS no longer provides unscheduled care - we provide treatment. Care is with dignity - that's impossible for our patients in corridors.

The model of acute care is not working or sustainable. We need a completely different approach - with long term investment in prevention and community care

111 need to change risk thresholds as far too many come to a&e when they don't need to.

There needs to be alternative pathways to get care overnight for those who don't have accidents or emergencies. A&E shouldn't be anything and everything

Staff are broke - supporting staff is just as vital as looking after patients

This government have totally decimated our the NHS. They are the ones who should be apologising to our patients not our staff.

Hundreds are dying each week because of lack of hospital capacity and corridor care in a&e, yet it's not on the news and no public uproar. As staff and patients we have accepted the unacceptable.

Everyone needs to know the reality of what is happening - so that politicians offer a different solution and patients get the care they deserve. Sadly most of the public have no idea of what is genuinely going on.

And finally, a post night fry up is the best antidote for the stresses of the weekend. Goodnight!

Most of the medical staff and patients had left Al Shifa hospital, as the Israeli occupation army ordered everyone to evacuate the hospital. Many patients can not leave the hospital as they are in the ICU beds or the baby incubators.

I along with 5 other doctors, will stay at Alshifa hospital with 120 patients (as patients can not leave due to their health conditions). We call the @ICRC @ICRC_ilot @WHO to take action to protect the medical staff and patients at Al Shifa hospital.

Dr Ahmed Sabra, a British national and Consultant Interventional Cardiologist at Mid Yorkshire Hospitals Trust is stranded in Gaza.

His wife and children who were with him have been allowed entry to Egypt but had to leave him at the border as his was not granted entry. He has been sent back into Gaza, where his life is constantly at stake.

The traumatic experiences he has had over the last 4 weeks are unimaginable to us in the UK - there are too many to list in this post. His daughter’s online post is attached. I cannot imagine the pain they felt at the border; not knowing if they would ever meet again. Simply heartbreaking.

He is a British national. One who has served his country’s National Health Service for over a decade. He has given his all through a pandemic, and continues to provides first class emergency care at all hours of day and night to those who suffer heart attacks in West Yorkshire.

After all his efforts, he needs his country now more than ever before. Our dear friend and esteemed colleague must be evacuated from Gaza NOW, be reunited with his family and allowed to return home safely. I urge the United Kindgom to act with the highest priority and if anyone can provide him any assistance I would be most grateful to them.
