1. Resuscitate First
NS 15-20 mL/kg in hour 1.
DKA = dehydration + acidosis. Fix volume before anything else.

2. Start Insulin, But Smart
Regular insulin 0.1 U/kg/hr IV infusion.
No bolus if K+ <3.3 or if patient is hypotensive.

3. Watch Potassium Like a Hawk
K+ <3.3 → Hold insulin, give K+ first.
K+ 3.3-5.2 → Add K+ to fluids.
K+ >5.2 → Hold K+, start insulin.
Most DKA deaths = from K+, not glucose.

4. Fluids After Hour 1
Switch to 0.45% NS once BP stable.
Add D5 when glucose <200-250 mg/dL. Never stop insulin early or you’ll get rebound ketosis.

5. Find the Trigger
Infection, missed insulin, MI, steroids, new DM.
Treat the cause or they’ll bounce back.

6. Monitor, Monitor, Monitor
VBG/Bicarb, K+, glucose q1h.
Neuro checks q1h. Cerebral edema is rare but deadly.

Key lesson I learned the hard way: Don’t chase the bicarb. Close the anion gap.

Question for the timeline: Why? What is the exact pathophysiology that makes immediate IV insulin so dangerous in this scenario and what MUST you push first?

The intern’s instinct is understandable—glucose is 650 mg/dL—but the first life-threatening problem in severe DKA is usually not hyperglycemia. It is profound intravascular volume depletion and electrolyte derangement.

When IV insulin is given its cause -
1. Glucose rapidly enters cells
2. Potassium rapidly shifts intracellularly
3. Serum K⁺ may fall precipitously
4. Severe hypokalemia can develop
5. Result:
* Ventricular arrhythmias
* Cardiac arrest
* Respiratory muscle weakness

What MUST be given first?

1. Aggressive IV fluids (first priority)

0.9% Normal Saline

* Restores perfusion
* Improves renal blood flow
* Lowers glucose even before insulin
* Begins clearance of ketones

2. Check potassium immediately

<3.3 mEq/L -Hold insulin, give K⁺ first

3.3–5.2 mEq/L-Give K⁺ with fluids and start insulin

>5.2 mEq/L-Start insulin, monitor K⁺ closely

Metronidazole inhibits the metabolism of warfarin (via CYP450 inhibition), causing warfarin levels to rise and the INR to increase, significantly increasing the risk of bleeding.

Clinical clue:-
A patient stable on warfarin who develops easy bruising, gum bleeding, epistaxis, or GI bleeding shortly after starting an antibiotic should raise suspicion for a drug interaction.
