@projsolve We do it on a daily basis, really -- on rounds. The medical student presents, the intern adds details, and the resident/fellow/attending build off that. Iterative thinking! Can be incorporated into team huddles, etc.
@projsolve Bureaucracy can easily get in the way of patient care, as evidenced by the writer's experiences with some of those removed from his situation. Those of us in medicine see a bit of this every day. @solvechat
