🚨 WEEKLY AI HALLUCINATION / AGENT FAIL CHECK!

When an AI agent gets something wrong, who takes responsibility: the AI, its developer, or the company deploying it?

This is no longer just a theoretical question. We have already seen real businesses experience the consequences of trusting AI systems without sufficient safeguards.

Consider what happened to Air Canada in 2024.

In November 2022, a passenger named Jake Moffatt used Air Canada's website chatbot to ask about bereavement fares after the death of his grandmother.

The chatbot informed him that he could apply for a bereavement discount within 90 days, even after completing his journey.

Trusting that information, he purchased his flight tickets.

There was just one problem: the chatbot's answer contradicted Air Canada's actual policy, which required passengers to apply for the discount before travelling.

When Moffatt requested his refund, the airline refused. The matter eventually reached British Columbia's Civil Resolution Tribunal.

On February 14, 2024, the tribunal found Air Canada responsible for negligent misrepresentation and ordered it to pay compensation.

The total award, including damages, interest and fees, was C$812.02.

This incident highlights a much bigger issue with AI deployment.

An AI system can sound confident, provide detailed explanations and still deliver completely incorrect information. When businesses allow such systems to interact with customers or make operational decisions, a single unchecked response can create financial, legal and reputational consequences.

So, where could an entry Circuit Breaker have helped?

Imagine an AI system operating with a simple safety mechanism:

Verification: Before delivering important policy information, the agent checks its response against an approved and authoritative source.

Contradiction detection: If its generated answer conflicts with the verified information, the system flags the inconsistency.

Automatic interruption: Instead of confidently delivering potentially misleading information, the Circuit Breaker stops the response.

Human escalation: The issue is forwarded to a human operator for confirmation before the customer receives an answer.

Incident logging: The failed response is recorded for investigation, monitoring and future improvements.

The goal isn't to prevent AI from making every mistake. It is to prevent an unchecked mistake from becoming a real-world incident.
