The subependymal zone has a quiet story to tell.
A niche of adult neural stem cells, sustained by long, solitary medullary arteries—no backups, no collaterals. When radiation enters the scene, these vessels take the hit. Hypoxia follows. Enhancement appears later.
So when you spot new subependymal enhancement within 2 years after radiotherapy, pause the alarm bells.
Many a times it’s pseudoprogression.
