𝗖𝗼𝗵𝗼𝗿𝘁 𝘀𝘁𝘂𝗱𝘆 𝗼𝗳 𝘁𝗵𝗲 𝘄𝗲𝗲𝗸 - The study, “𝗣𝗿𝗲𝗰𝗹𝗶𝗻𝗶𝗰𝗮𝗹 𝗣𝗿𝗼𝘁𝗲𝗶𝗻 𝗦𝗶𝗴𝗻𝗮𝘁𝘂𝗿𝗲𝘀 𝗼𝗳 𝗖𝗿𝗼𝗵𝗻’𝘀 𝗗𝗶𝘀𝗲𝗮𝘀𝗲 𝗮𝗻𝗱 𝗨𝗹𝗰𝗲𝗿𝗮𝘁𝗶𝘃𝗲 𝗖𝗼𝗹𝗶𝘁𝗶𝘀: 𝗔 𝗡𝗲𝘀𝘁𝗲𝗱 𝗖𝗮𝘀𝗲-𝗖𝗼𝗻𝘁𝗿𝗼𝗹 𝗦𝘁𝘂𝗱𝘆 𝗪𝗶𝘁𝗵𝗶𝗻 𝗟𝗮𝗿𝗴𝗲 𝗣𝗼𝗽𝘂𝗹𝗮𝘁𝗶𝗼𝗻-𝗕𝗮𝘀𝗲𝗱 𝗖𝗼𝗵𝗼𝗿𝘁𝘀”, published in Gastroenterology provides compelling evidence for the existence of blood-based proteomic biomarkers that precede clinical diagnosis of inflammatory bowel diseases (IBD). Led by first author Dr. Oscar Grännö and corresponding author @jonashalfvarson , the international, multicenter research effort analyzed proteomic profiles from large-scale biobanks across Sweden.

𝗞𝗲𝘆 𝗙𝗶𝗻𝗱𝗶𝗻𝗴𝘀:

𝗣𝗿𝗲𝗱𝗶𝗰𝘁𝗶𝘃𝗲 𝗣𝗿𝗼𝘁𝗲𝗶𝗻𝘀 𝗳𝗼𝗿 𝗖𝗿𝗼𝗵𝗻’𝘀 𝗗𝗶𝘀𝗲𝗮𝘀𝗲 (𝗖𝗗): A 29-protein plasma signature accurately predicted future CD cases. The model showed strong performance in discovery (AUC 0.85), validation (AUC 0.87), and inception (AUC 1.0) cohorts. Proteins were enriched in gut barrier and macrophage-related pathways, indicating early immune involvement.

𝗠𝗼𝗱𝗲𝗿𝗮𝘁𝗲 𝗔𝗰𝗰𝘂𝗿𝗮𝗰𝘆 𝗳𝗼𝗿 𝗨𝗹𝗰𝗲𝗿𝗮𝘁𝗶𝘃𝗲 𝗖𝗼𝗹𝗶𝘁𝗶𝘀 (𝗨𝗖): Protein markers also predicted UC but with lower accuracy (AUCs 0.77 in discovery and 0.67–0.95 in validation). The results suggest both overlapping and unique early biological pathways for UC versus CD.

𝗟𝗼𝗻𝗴 𝗣𝗿𝗲𝗰𝗹𝗶𝗻𝗶𝗰𝗮𝗹 𝗪𝗶𝗻𝗱𝗼𝘄: Proteomic changes were detectable up to 16 years before diagnosis, supporting a prolonged subclinical phase and the possibility for early detection and intervention.

𝗧𝗮𝗸𝗲 𝗵𝗼𝗺𝗲: This landmark study provides strong evidence that preclinical changes in blood-based protein expression can reliably predict the future onset of Crohn’s disease and to a lesser extent, ulcerative colitis many years before symptoms appear. These findings support the potential for a proteomics-based screening approach for at-risk populations, laying the groundwork for earlier diagnosis with execution of interventional strategies in IBD.
