In a pt with ILD, your doc may suspect myositis if any of the following are seen:
💫Raynaud's phenomenon (fingers becoming numb&pale in the cold)
💫Skin rashes
💫Mechanic's hands
💫Muscle weakness
💫Joint pains
Lab tests may be done for enzyme levels.

- Anti-Synthetase Ab (80-90% ILD)
- Anti-MDA5 (50% ILD in US)
- Overlap Ab (PM-Scl, Ku, Ro52/SSA and U1RNP - 40-70% ILD)

Rapidly progressive ILD is a rare manifestation in #myositis, with fast progression over weeks to months.
💫Common in pt with anti-MDA5 & occasionally in Anti-Synthetase & overlap Myositis.
💫Associated with palmar papules, amyopathic DM & poor survival.
People with inclusion body myositis (IBM) who are hospitalized are at risk of complications caused by difficulty swallowing, which can lead to aspiration pneumonia, and falls.

#MyositisPearls for Patients 44- Doctors should consider Myositis and order Myositis Antibodies in patients with shortness of breath or cough due to ILD because ILD alone can be the first sign of Myositis; especially in Anti synthetase syndrome (a type of Myositis).

Skin biopsy may not be required in a typical DM case with classical rashes. However,in case of less typical DM rashes, it may be used for confirmation. Note lupus and DM skin biopsy looks a like.
#MyositisPearls for Patients 43-
Skin biopsy is required for the rashes of DM - where it is used for confirmation of the diagnosis & rule out other skin problems. However it may not be required in a classic DM rashes with classical symptoms.
