💡Interesting work comparing HV-HDF and HDx using different β2-microglobulin clearance metrics.

✅As expected, HV-HDF achieved higher β2M clearance.

✅The novel aspect was the evaluation of eCDCβ2M, an equivalent continuous clearance metric that may better reflect middle-molecule kinetics than traditional reduction ratios or urea-derived measures.

The next challenge: proving that these metrics predict clinical outcomes, not just toxin removal.

Extracellular vesicles are released from all cells. They play roles in antigen presentation, immune regulation, and metabolic signaling.

Biology & Measurement
Can be measured in blood but not in urine
Appear after ischemia and are linked to ferroptosis, a type of cell death
Involved in both induction of immune response and induction of immune tolerance

Role in Transplant Rejection
Donor-derived extracellular vesicles cause B cell activation and donor specific antibody production
Complement system activation has been shown through extracellular vesicles
In acute kidney injury of the graft, aquaporin 2 release has been demonstrated via extracellular vesicles
Complement activation can be detected using extracellular vesicles

Role in Autoimmune Kidney Disease
Carry autoantigen-antibody complexes in autoimmune kidney diseases
Associated with glomerulosclerosis

Takeaway: Extracellular vesicles act as messengers between cells in transplant rejection and kidney disease. They link immune activation, complement activation, and tissue injury. Potential biomarkers and therapeutic targets.
