@FCAFigueiredo @WilliamYHTW I agree. After enucleation, I close larger vessels with a bipolar diathermy with a ball. For the patient, safety, and for me, peace of mind!

@DongNguyeb In my experience, 2J is even a bit too much. 1.5J-1.7J is optimal, higher energy tends to be too aggressive, especially near mucosa.

A handful of papers have shown that transperineal biopsy yields more cancer than the transrectal approach. One possibility is superior anterior sampling, but a simpler explanation is improved sampling density of the peripheral zone. See for yourself:

@DongNguyeb Flaking and coat traction is under better control. Flaking should be along the grain of the capsula, so this double loop will be a problem.
