@nickmmark Agree 100%, this is unacceptable. Usually standardized compounding softwares such as Dose Edge force pharmacists to visually inspect photos of what techs compounded to make sure the right drug was put into a syringe. Can’t imagine how this got mixed up
Obviously ready-to-use products are safer and preferable. But there should be procedures to make this kind of error impossible.
Things like standardized compounding workflows, independent verification by a second person, and using technology like barcode scanners are important.
High-risk meds like contracted electrolytes or neuraxial meds should get extra close scrutiny. (And extra labeling!)

Due to a mixup in the Ascension hospital pharmacy, FOUR patients received potassium instead of bupivicaine for epidurals before routine joint surgeries. At least one is now paralyzed.
The worst error I’ve ever heard of. Hard to see how this could happen.

🧪New study exploring albumin binding in salicylate overdose—very cool work. Albumin ↓ free salicylate in vitro; largest drops required high levels, but simply correcting low albumin was intriguing. Mechanistic, hypothesis-generating. First bench-top study for me! PMID: 41413367
