I had the privilege of meeting with Dr. Arrighi and Joyce Chang Price for more than an hour to update them on the work that we are doing in Project IMG , and discuss several important issues affecting graduate medical education in the United States and globally. We spoke about the challenges residents and fellows are facing, the ways ACGME is supporting learners at both the national and international levels, and potential areas for future collaboration with Project IMG.
There are exciting conversations in progress, and we look forward to sharing more soon.
On a personal note, Dr. Arrighi is inspiring for me, as he completed his cardiology fellowship at The National Institutes of Health @NIH, the same institution where I will soon begin my endocrinology fellowship. Seeing his remarkable career trajectory and current leadership as President and CEO of ACGME-I is truly motivating.
It was also wonderful to reconnect with Dr. Stuart Slavin, with whom we have been working around the transition from applicant to resident and the well-being challenges that many trainees face during this process.
The last time I got to meet with many of ACGME leadership was last year in Thailand during the World Federation for Medical Education conference, where I represented Project IMG as a delegate.
Great things are in progress, and we are excited for what is ahead.
Tomorrow, I will be back at the ACGME headquarters to meet with the Rural and Underserved Communities department, and I will also visit the ACCME headquarters to meet with Dr. Graham McMahon, President and CEO of Accreditation Council for Continuing Medical Education (ACCME), who has been a strong supporter of Project IMG and attended our most recent IMG event.
Grateful for these conversations, the continued support, and the opportunity to keep building bridges for international medical graduates, residents, fellows, and physicians around the world.

๐จ BREAKING NEWS: Federal Judge Vacates $100,000 H-1B Visa Fee
A federal judge in Massachusetts has vacated President Donald Trumpโs policy imposing a $100,000 fee on employers filing new H-1B visa applications.
U.S. District Judge Leo Sorokin ruled that the policy violated the federal Administrative Procedure Act and the Constitution. The court agreed with the 20 states that challenged the policy, finding that the $100,000 payment functioned as a tax โ and that Congress had not delegated that taxing power to the executive branch.
The Trump administration has said it will appeal the ruling.
The H-1B visa program, created in 1990, allows U.S. employers to hire highly skilled international professionals in specialty occupations. Before this policy, H-1B application fees were generally reported to range from approximately $2,000 to $5,000 per application.
For international physicians, hospitals, universities, researchers, and healthcare institutions, this ruling matters.
A $100,000 fee could have created a major financial barrier for employers seeking to recruit international medical graduates, researchers, and skilled professionals who contribute to the U.S. healthcare system.
Immigration policy is physician workforce policy.
It is hospital staffing policy.
It is patient access policy.
It is healthcare equity policy.
At a time when the United States continues to face physician shortages, especially in underserved communities, policies that limit access to qualified international physicians can directly affect patients and the healthcare workforce.
Project IMG will continue to monitor this decision, the expected appeal, and its impact on international physicians, trainees, hospitals, universities, and the future of the U.S. healthcare system.

UPDATE: Most Immigrants May Not Need to Leave the U.S. to Get Green Cards โ But Confusion Remains
Recent reports created concern among immigrant communities after a USCIS policy memo suggested that some individuals applying for permanent residency may need to leave the United States and continue the process from abroad, except in limited circumstances.
DHS has since clarified that this is not a blanket change and that many applicants may still be able to remain in the U.S. while their cases are reviewed. However, important details remain unclear, including who may be affected, how pending cases will be handled, and how individual determinations will be made.
For international medical graduates, physicians, residents, fellows, researchers, and healthcare workers, immigration clarity is important. Many are serving patients, supporting hospitals, advancing research, and contributing to communities across the country.
