Aftercare:
Daily meatal hygiene
Keep the bag below bladder level
Remove as early as possible
Document date, size, balloon volume, urine volume and colour

Tips:
Can't see the meatus? Look just above the vaginal opening. It may be retracted in postmenopausal women.
Catheter went into the vagina by mistake? Leave it in place as a landmark and use a fresh sterile catheter.
Use a smaller catheter if resistance is felt. Never force.

Complications:
Immediate. Pain, urethral trauma, false passage, bleeding.
Early. UTI (the commonest), bladder spasm.
Late. Encrustation, blockage, urethral stricture, bladder stones.

Procedure:
Explain, get consent, ensure privacy, and have a chaperone.
Wash hands. Position supine, knees flexed, hips abducted (frog-leg).
Open the sterile pack. Put on sterile gloves.
Drape the patient. Expose the perineum.
Separate the labia with your non-dominant hand. This hand stays "dirty" for the whole procedure.
Clean the meatus with antiseptic, front to back. One swab per stroke.
Lubricate the catheter tip.
Insert gently into the urethra, about 5-6 cm, until urine flows.
Advance a further 2-3 cm.
Inflate the balloon with 10 mL sterile water. Never inflate before urine is seen.
Withdraw gently until you feel resistance.
Connect to the drainage bag. Secure to the inner thigh.
Reposition the labia. Document

10 mL syringe with sterile water
Antiseptic (chlorhexidine or normal saline)
Water-based lubricant gel (sterile)
Urine bag, kidney dish
Good light, tape or securing device

Suspected urethral injury (blood at meatus, pelvic fracture)
Active UTI, unless essential
Patient refusal
