This study reports a statistically significant association between a larger appendicolith diameter and complicated acute appendicitis, but no cut-off value for appendiceal diameter for the increased risk of complicated appendicitis could be determined and even in the group of patients with appendicoliths having a diameter of 1–3 mm almost half of the patients had complicated appendicitis.
