A good example of a borderline phyllodes tumor. 50 yo woman, 5 cm mass. Infiltrative border, moderate stromal cellularity, mild atypia, up to 8 mitoses/10 hpf. No stromal overgrowth, no malignant heterologous element. @TheISBP@EdiBrogi @fcschmitt @AVincentSalomon@LizaMQuintana
Head-scratcher Tuesday.
This consult case befuddled me for a long time until I did one thing.
Here’s the story it came with. Latin American lady with a retroareolar mass, possibly painful but unsure, slow growing over the past 2-3 years.
Let the guesses and work-ups begin!

One of our brilliant residents showed me this case. Supraclavicular lymph node. No other clinical information at the moment.
Micropapillary to papillary structures, some mitoses, some mucin production.
(Short thread follows):
Beautiful example of superficial lamina propria invasion in a papillary urothelial carcinoma courtesy of @slusagar demonstrating:
- small, somewhat irregular nests
- retraction artifact
- loss of capillary/vascular cuff
