Today I had a surgery denied. The denial letter gave me a # to call for an appeal at @WellcarePlans
I called the number - on hold way too long. Then someone answered, asked all the information, numbers, story, etc. Then told me she couldn’t help me and to call another number
I get asked all the time why I, as a urologist, talk abt vaginal estrogen. Let me tell you. I was practicing as an attending for 5 years in a large managed care organization. Two of my most common referrals were:
1) post menopausal woman w recurrent UTI
2) " " " w micro hematuria
All I see here is:
incidentaloma, incidentaloma, incidentaloma…
What’s an incidentaloma?
A ditzle / wart / spot that may or may not be concerning, but ends up triggering a path of follow up scans ($), invasive/procedural diagnostics like biopsies and surgeries ($$$$$), complications of those invasive diagnostics (bleeding, infection, death), and anxiety and cost.
Add on the concept of “pre-test probability”, and elective full body scans can only yield higher false positive rates.
Based on existing studies in radiology literature for non-elective scans (aka, they were looking for some pathology based on risk factors or symptoms of the patients), here’s some stats:
🏥~40% of imaging results in incidental findings (incidentalomas).
🏥Of those, only 1.1% were actually clinically significant and/or cancer.
The rest is just $$$, complications, and anxiety…
Knowledge is power, but that power can be dangerous…

BREAKING: The CEO of @Hertz wrote a letter to Puerto Rico‘s representative in Congress stating that Hertz has rewritten its policy to make it abundantly clear that Puerto Rico is a U.S. territory - not a foreign country - & because of that Puerto Rico driver’s licenses are not international IDs they are just as valid as any ID from any state in this union. This news follows the story I first reported last week, where a Puerto Rican man at the New Orleans international airport was denied a rental car because at least two Hertz workers said his license was international. On top of that, the man who recorded the incident, had the police called on him by a Hertz employee who said he can never rent from Hertz again because he recorded the incident. *She was wrong; he can rent from Hertz again*. The police officer who responded to the call was dismissive and rude and is now the subject of an internal investigation by his police department because the customer, Mr. Humberto Marchand, filed a complaint with the @KennerPolice. Following our report, Representative @Jenniffer wrote a letter to Hertz asking for a companywide educational campaign, and she got it.
