In PROSPECT II, elevated TC, LDL-C, & non-HDL-C were strongly associated w/ pancoronary atherosclerosis & lipid deposition, whereas elevated Lp(a) was strongly associated w/ the presence of focal vulnerable plaques.

First day at #ACC25, here are the results of 10 trials presented today. I counted 3 NEJM, 1 Lancet, and 1 JAMA.
WARRIOR: Intensive medical therapy did not reduce major CV events at five years in women with suspected INOCA.
STRIDE: Semaglutide improved walking distance, symptoms, and quality of life in PAD with diabetes.
API-CAT: Low-dose apixaban was noninferior to full-dose for preventing VTE recurrence in cancer patients.
RIVAWAR: Rivaroxaban was as effective and safe as warfarin for post-MI LV thrombus resolution at three months.
FARES-II: Four-factor PCC was superior to frozen plasma for bleeding control in cardiac surgery.
REVERSE-IT: Bentracimab reversed ticagrelor’s antiplatelet effects in urgent surgery or major bleeding.
HOST-BR: Three-month DAPT reduced adverse events without more bleeding vs. one-month in high-bleeding-risk patients post-stent.
SOUL: Oral semaglutide reduced MACE by 14% over four years with no safety concerns.
ADVANCE-HTN: Lorundrostat significantly lowered systolic BP at 12 weeks in uncontrolled hypertension.
DAPA-TAVI: Dapagliflozin reduced mortality and heart failure worsening post-TAVI vs. standard care.

The first EuroIntervention issue of 2025 features a clinical consensus statement from the EAPCI, in collaboration with the @escardio Working Group on Cardiovascular Surgery, outlining procedural and technical requirements to enhance the efficacy and safety of PCI in high-risk patients on optimal medical therapy.
These patients, deemed unsuitable for CABG by the Heart Team due to unacceptable surgical risks, may benefit from PCI for prognostic reasons (e.g., left main disease, last remaining vessel, or extensive ischemia). The statement emphasizes the importance of a multidisciplinary decision-making approach involving interventional cardiologists, cardiac surgeons, anesthetists, and heart failure experts to weigh the potential risks and futility of PCI.

Are all stents the same for high bleeding risk (HBR) patients? BioFreedom, Resolute Onyx, Synergy, and Xience are approved for HBR patients by both the European Commission and FDA. While Orsiro Mission has strong evidence for HBR use, it lacks specific HBR regulatory approval. Ultimaster and Supraflex Cruz are not specifically labeled for HBR but are approved for short DAPT durations. We wrote a brief article on the topic of stent selection for HBR patients.
