CT scans in complex, undifferentiated critically ill patients save lives. we don’t have a mega-RCT to show this (analagous to CT screening for lung cancer) but it’s still true 😛
Please order the CT with IV contrast.

It's whatever diagnosis we are missing with the non-contrast CT that will damage the kidneys.
