This is a rare condition called Hematidrosis.
Under extreme emotional panic or fear, a massive adrenaline spike causes the tiny blood vessels feeding your sweat glands to violently constrict and then literally burst under the pressure. The blood leaks directly into the sweat glands.
As the body sweats from the intense panic, it pushes out a mix of water and actual blood. It looks terrifying and fatal, but it stops when the anxiety does. You don't treat it with bandages or surgery....you treat it with beta-blockers and anti-anxiety meds to shut down the adrenaline!
You hit your head hard and get a giant hematoma (a blood knot under the skin).
What should you NEVER take for pain?
A) Cold water

D) Hot tea
Many people make this mistake that can make bleeding worse.
I am Dr. Priyam and this actually takes me back to a case I encountered during my first year of residency.
Technically this is food poisoning. But...did you notice I didn't mention a single normal GI symptom like nausea, vomiting or diarrhea?
That's what makes this different and something to be learned separately. This is poisoning by a bacteria called Clostridium botulinum (Botulism).
Potatoes grow in dirt, which naturally carries the spores of this bacteria. You wrap it in foil and leave it sitting on the counter overnight.
In our Indian climate especially, that foil creates the perfect trap: zero oxygen, locked-in moisture and a cozy incubator. The spores hatch and pump out a deadly nerve poison while you sleep.
"But Dr. Priyam, didn't the microwave kill it?"
Here is the catch. The poison is heat-sensitive, but you have to actively boil it for a solid 10 minutes to destroy it. A quick two-minute zap just warms the food up unevenly. It doesn't get nearly hot enough, for long enough, to kill the poison.
The fix is simple: never leave foil-wrapped baked potatoes sitting out on the counter. Take the foil off and put them in the fridge!

It's B) Ibuprofen (along with other common NSAIDs like Diclofenac, Naproxen & Aceclofenac that also damage the kidney).
Think about what it actually does inside your body. Ibuprofen blocks prostaglandins. Your kidneys completely rely on these to keep their blood vessels wide open. When you take Ibuprofen frequently, those vessels constrict and literally choke off the blood supply to the kidneys. That’s how you end up with acute kidney injury.
The other options hit completely different organs:
1. Paracetamol (acetaminophen) is processed in the liver. High doses cause liver toxicity, not kidney failure.
2. Aspirin attacks the stomach lining and causes severe ulcers or GI bleeding.
3. Tramadol acts on the central nervous system, so the main risks are seizures and dependence.
This is exactly why we have to do a proper clinical risk analysis based on patient history. Never just take OTC painkillers blindly....always get them prescribed.

Refeeding syndrome (RFS) is a potentially fatal metabolic complication that occurs when nutritional support (either food, tube feeding, or IV nutrition) is reintroduced too quickly to someone who is severely malnourished or has been starving.
During prolonged starvation, the body shifts its primary fuel source from carbohydrates to fat and protein (ketosis). Intracellular minerals—specifically phosphate, potassium, and magnesium—become severely depleted, though blood levels might still look normal because the body tries to maintain equilibrium in the bloodstream.
When food (especially carbohydrates) is reintroduced:
Insulin Spike: The sudden influx of glucose triggers a massive release of insulin.
Intracellular Shift: Insulin drives glucose, potassium, magnesium, and phosphate out of the blood and into the cells.
Severe Blood Depletion: Because the body’s overall stores of these minerals were already depleted, this sudden shift causes serum levels to crash.

What happens during a 7-day water fast?
During prolonged fasting:
Insulin levels stay very low.
The body switches to burning fat and ketones for energy.
Total body stores of phosphate, potassium, and magnesium become depleted, even if blood levels initially appear normal because these minerals move out of cells.
Then comes the pizza and soda…
A meal high in carbohydrates (pizza crust + sugary soda) causes a large insulin surge.
Insulin rapidly drives:
Glucose into cells
Phosphate into cells
Potassium into cells
Magnesium into cells
This can cause a sudden fall in blood levels of these electrolytes.
Phosphate is needed to produce ATP, the body’s energy currency.
Severe hypophosphatemia leads to:
Weakness of the diaphragm → difficulty breathing
Heart muscle dysfunction
Reduced oxygen delivery (due to low 2,3-BPG in red blood cells)

Why the cardiac arrest?
At the same time:
Hypokalemia and hypomagnesemia make the heart electrically unstable.
This can trigger dangerous ventricular arrhythmias (such as ventricular tachycardia or ventricular fibrillation), leading to sudden cardiac arrest.
This is called Refeeding Syndrome

Step seven: Combustion stays outside. Generators, grills, camp stoves. Carbon monoxide kills more people in blackouts than anything else. This one is life or death.
Step eight: Run dark and quiet. Don't be the brightest, loudest house on the street. That makes you the most interesting house. Generator during the day, blackout curtains at night.
Step nine: Get comms up. Cell towers may fail within a day or two. Battery radio for info in, GMRS for local family communication out.
Step ten: In a big blackout, nobody is coming to help you. Plan accordingly.
A few things worth knowing:
Blackouts historically last hours to a few days, but the 2003 Northeast blackout affected 55 million people and the 2021 Texas grid failure killed over 200 people. Big events do happen. And likely will happen with more frequency.
A whole house generator is great, but a small portable generator with 3 to 5 gallons of stabilized fuel plus a portable power station covers most scenarios and doesn't announce itself to the whole neighborhood.
Water heaters hold 40 to 80 gallons of usable water. Learn how to drain yours from the bottom valve. That's a free reservoir most people forget.
Practice this before you need it. A weekend without power at home tells you exactly where your gaps are.
