2-The elevated LVOT VTI is either due to associated AR or because the PW sample was close to the orifice
2-Are you sure the planimetry done at the tip of AV?
@MahamadAboESoud I agree to some extent. I have seen one case with severe MR due to ruptured chorde and dominant A velocity in mitral inflow. Impaired relaxation makes severe MR unlikely but not always
