@albertoortegana Impossible to diagnose hydatid from the xray. History helpful. No collapsed cyst seen although obviously ‘cyst’ has drained into an airway??

Many are based on history and strongly held.
The commonest I regularly discuss is ‘ letting the air in’ which is based on the pre CT days of lung cancer surgery when the open/close rate was 50%

Unfortunately it never rewards the surgeon who has to deal with “all the difficult ones” but the reward comes with professional satisfaction and respect from others

I tell patients I have a 25% recurrence rate- maybe it might be less now!!
@_johnedwards Congratulations on a well developed and thought out repair. I do think you need to mention that the posterior extent of rib splaying is often more than anticipated and needs attention well behind the observed hernia.

We have an ‘adult guardian’ default decision maker who is legislated to make this decision. That said they might look at the soft evidence in this circumstance (they must have a bad head injury) and make the decision is to wait🤷🏻♂️
