@SEHCResearch & @UWaterlooSPHS scientists are leading a new @CIHR_IRSC funded project focused on developing practice resources to support more structured integration of personal support providers & their care contributions in home & community care teams!
Through more meaningful and intentional inclusion of PS providers in team-based home care delivery, project outputs will amplify PS #leadership in the delivery of #PersonCentred home care, improving #ProviderExperience & contributing to stabilization of this essential #workforce.
Key findings from my perspective? It’s not as much about the tool (we know that it is rigorously developed and tested) but rather HOW we integrate it into daily practice. This must be intentional and involve co-design of processes AND education / resources to support use.

In this study we used group concept mapping to collaboratively identify action areas and co-develop strategies to support use of the #interRAI Check Up to support #integrated person-centres health and social care for #OlderAdults and their #CarePartners.
A study compared 3 #FallRisk Ax tools from 18 #LTC homes in NS & NB- A history of falls was top predictor of future falls. Collecting more factors to predict fall risk does not add greater value. #interRAI Fall Risk CAP stood out as the most reliable tool.
