@david__highton@twpainter All major surgical procedures do not lead to significant blood risks so need to choose TXA wisely (if bleeding risk is significant enough or microhemostasis is a problem etc.) These risks are multi factorial.
@ChowdhuryTumul@david__highton But this is impossible to tell beforehand and we have shown there is no downside to using TXA. We also know that blood transfusion is a scarce supply. So why not use a safe and proven drug to prevent it? TXA is safe, inexpensive and proven in all major areas of surgery. @NEJM
@david__highton Yes I believe we should. Clear evidence now of reduced bleeding and non-inferiority wrt thrombosis. Lots of secondary analyses yet to come!
@NEJM I believe it should be standard of care in at risk patients undergoing major surgery. The incidence of significant bleeding is often vastly underestimated as is its impact on morbidity and mortality. We also now have evidence that it is non-inferior with regards to thrombosis
