Full technical reconstruction of both incidents, root cause analysis, and MITRE ATT&CK TTP mapping across every observed Lazarus Group technique used in April 2026.
Five production grade security checklists covering every layer of the stack:
Protocol Security: smart contract invariants, DVN multi verifier config, oracle staleness and cross source consensus, governance timelocks
Node Infrastructure Security: RPC binary integrity monitoring, remote signer architecture, network segmentation, multi provider consensus clients
OS Security: Linux and macOS hardening scripts for node operators and signing workstations, HSM key management, Shamir Secret Sharing for seed backup
DApp Security: CSP and SRI headers, transaction simulation before sign, external code sandbox policy, secure multi provider RPC client
Operational Checklists: pre launch, monthly ops, incident response across 5 minute, 30 minute, and 48 hour phases, and developer OPSEC quick reference
Every checklist item traces directly to a confirmed failure in the April incidents. This is not generic security theater.
The $578M lost in weeks was not primarily a smart contract problem. Both protocols had clean audits. The failures were at the infrastructure, governance, and human layer. Audits do not test those.
Our goal is for this repository to become the go to security reference that protocol teams bookmark, follow, and contribute to. The more teams that adopt these controls, the harder it becomes for state sponsored actors to repeat what just happened.
