It is diagnostic of Wellens'. We had to go seek out to see if there had be a prehospital ECG. There was. Wellens' really is "Transient STEMI or OMI" with spontaneous reperfusion. You can't find a case to refute this.

Prehospital ECG is a valuable diagnostic tool that is often lost during ED evaluation of chest pain and there is nothing clinically more valuable for diagnosing ACS than serial ECGs. Our recent work published in Annals of EM confirms this, check it out!
