Post surgical gastrocutaneous fistula.
I really think it’s critical to rough up the fistula to de-epithelialize as much as possible before closure is attempted.
I use brush (a lot of reps!) and spray coagulation at the internal orifice.

Let’s walk through a serious UGIB bleeding:
Patient with acute UGIB, melena, hemodynamic instability is resuscitated and endoscopy performed urgently.
This large clot is seen in duodenum along with fresh blood. Uncertain if active bleeding.
